Class Templates
Use these templates to prepare for your class.
- Cover Sheet – Use this as a cover sheet for the information packet for each class. It provides visual consistency for the program, signifies date/time/and place class occurs and has pertinent contact information included. Behind it include the agenda, any handouts you decide to use, and copies of the recipes that will be made in class.
- Agenda – Use this for each class to help the students know what to expect and to serve as a guide for each class.
- Instructor Script – This is for the instructor only, not to be distributed to participants. This serves as a guide for the educator to stay on track during class if needed.
- Class Checklist – This is for the instructor only, not to be distributed to participants. This can serve as a shopping list as well as a reminders list to help the educator prepare for the class.
- Sign In Sheet – This is used to keep record of participant attendance.
- Certificate – This is used to recognize participants who attend all of the classes in a series.
Recipe Templates
By using these templates for recipes used in class, you can provide visual and format consistency with recipes pulled from various sources.
- Recipe 1 – This template has dark green accents. Pictures of kale, feta cheeses, cinnamon, cranberries, and red pepper flakes are included.
- Recipe 2 – This template had dark red accents. Pictures of berries and granola are included.
- Recipe 3 – This template has teal accents. Pictures of mango chunks, honey, cranberries, and walnuts are included.
Waivers
Waivers are used to warn participants of potential hazards of the class and to provide a media release for use of photographs.
- Waiver for adults – English

Surveys
Surveys help evaluate the effectiveness of the program by measuring participant attitudes and behaviors before the first class and after the last class. Survey results can be calculated using simple spread sheets or a Qualtrics program.

These forms can be used by ANY EDUCATOR, not just those conducting Healthy South Texas and Healthy Texas projects. These surveys include standard questions as common indicators that are consistent across the Extension suite of health programs and contribute to the consistent overall outcomes of Extension Health programming.

These are some of the common recipes used in the pilot program and were found to work well in the mobile setting. You can use these in your classes as desired. Or, you can find other recipes from various sources and plug them into into the recipe templates above to provide a similar look.

- Class Preparation Timeline – This is a suggested timeline for planning and implementing classes. Not all class preparation will flow exactly like this, but this gives you an idea.
- Supply List – This is a suggested list of items you might want to have in the equipment tubs you take on-site for the classes.
- MCS Brochure/Promotional – This brochure can be used to recruit both hosts and participants
- MCS Flyer – This fillable flyer can be used to announce upcoming classes and has the same look as the class cover sheets providing visual consistency.
- Results Template -Once you have your survey results ready, you can plug them into this graphic to provide an attractive display of the program’s progress to help you share the results with others. Or, to customize the pieces of the template (order, size, arrangement, ect.) use this tool.
