Declaration of Eligibility

This is required for ALL 4-H events or activities from ALL 4-H members in public, private, and/or home school that requires a 4-H member to be absent from school.

A completed academic eligibility form is required before any 4-H youth member can participate in any competitive 4-H activity or participate in any 4-H event, competitive or not competitive, that requires school to be missed.

Eligibility is determined based off of the most recent closed grading period prior to an event.

Completed form must include:

• activity date(s)
• name of activity
• parent signature
• participants name
• Extension Agent signature
• school designee signature
