TCJLS – Eligibility Form

Dear Tarrant County 4-Her’s participating in TCJLS:

For those not aware, each participant in TCJLS should have a Declaration of Eligibility Form completed.

Texas 4-H is an officially recognized extracurricular activity and follows the “No Pass, No Play” UIL Guidelines. This includes public, private, and homeschool.  (For homeschooled youth only – parents will sign as the “Principal or Designee”.)

E-mail (PDF file only, NO JPEGs or Pictures please) your completed form to andrea.amador@ag.tamu.edu.

Deadline to be turned into Extension Office is Tuesday, February 28.  *Each youth is responsible for filling out and submitting their form.
