2023-2024 Declaration of Eligibility Form
Sent, Dear 4-H Members and Families, All participants will need to have an Academic Eligibility Form filled out and turned into the Tarrant County Extension Office. This form will allow you to participate in all events. You need to have a form for each event you are participating in.
