📌 New This Year:
All award applications have moved to Google Forms for easier access! Prefer a PDF? No problem—you can still email it to Dallasco4h@gmail.com. Just make sure to submit by July 14, 2025! Late submissions will not be accepted. (Please only submit either a Google or PDF form, not both)
🔍 Be sure to:
✅ Complete the correct age division form
✅ Follow all requirements
✅ Include a current photo (Email photos to Dallasco4h@gmail.com.)

Cloverbuds (Ages 5 to 7) can apply for an award using one of the following links.

Juniors (Ages 8 to 10) can submit their award applications using one of the following links: 2025 Junior Awards Application Fillable PDF – 2025 Junior Awards Application

Adult Leadership can be nominated for these categories: Outstanding Leader Award, Mentor of the Year, or an individual or business/stakeholder can be nominated for the Heart of 4-H Award. If selecting an individual, they cannot have a 4-H member participating in the current 4-H program year.
