FORT BEND COUNTY 4-H CANNOT CONTACT YOUR YOUTH&#8217;S SCHOOL TO DISCUSS YOUR YOUTH&#8217;S GRADES OR ACADEMIC INFORMATION.<\/strong><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul>\n<li><strong>FORT BEND COUNTY 4-H SIMPLY GAINS CONFIRMATION OF ACADEMIC ELIGIBILITY AND EXCUSED ABSENCES FOR INDICATED EVENTS ONLY.<\/strong><\/li>\n<\/ul>\n<p>A completed Form includes:<\/p>\n<ul>\n<li>Name, Grade, and School of Student<\/li>\n<li>Activity Name<\/li>\n<li>Date of Event<\/li>\n<li>Principal or Designee sign off<\/li>\n<\/ul>\n<p>A Form will need to be filled out for EACH event you attend\/participate. You can provide multiple events per form as it is good for a maximum of 30 days.<\/p>\n<p>Once completed, the form will need to be taken with you to the Event or Contest AND a copy of the form will need to be sent to the County Extension Office. The completed Form can be emailed, faxed, or dropped by the County Extension Office.<\/p>\n<p><span style=\"color: #ff0000;\"><strong>Fort Bend County 4-H only accepts and processes eligibility
