  • I do hereby consent and agree that it is satisfactory for all horses participating at the Brazoria County 4-H Horse Show be housed at the Brazoria County Fairgrounds. Furthermore, we do not hold any members and/or leaders of the Brazoria County, the Texas AgriLife Extension Service, Brazoria County 4-H or the Brazoria County Fair Association or any other persons connected with the housing of animals responsible for any bodily injury or any damage or loss sustained by the above mentioned animals that are to be housed.
  Release of Liability Acknowledgement Required.
• Requirement - Minor's Release*
  • I do hereby consent and agree that it is satisfactory for our minor son or daughter to compete in the Brazoria County 4-H Horse Shows and other activities. Furthermore, we do not hold any members and/or leaders of the Brazoria County 4-H Horse Group and the Texas Agricultural Extension Service or any other persons connected with the show responsible for any bodily injury or any damage or loss sustained by the above mentioned 4-H member while participating in any of these activities. In case of sudden illness or accident to the above named participant requiring immediate treatment or surgery while enroot to this activity, while there as a participant, and/or while returning from the event, I authorize Texas Agricultural Extension Service personnel serving as chaperon(s) to take such action as seems appropriate to protect the health and physical well-being of the participant.

• The following information is provided as an aid to the chaperon(s) in dealing with the well-being of the named person. The participant has the following health conditions (include such things as handicaps, diabetes, asthma, allergies, and medications needed):*
