This worksheet is a guide only.<br \/>\n2026 Waiver Form: <a href=\"https:\/\/d84-h.tamu.edu\/files\/2025\/03\/2025-Waiver-Indemity-Form.pdf\">PDF<\/a>\u00a0 \u00a0\u2013 <em><strong>AGAIN THIS YEAR! <\/strong>Signed Waiver form must be <strong>scanned as a pdf and uploaded onto 4-H Online<\/strong>\u00a0during the entry process.<\/em><\/p>\n<ul>\n<li><em> The 4-H member and parent may sign the waiver form, scan as a pdf and upload the form OR take the signed form to the Extension Office to be scanned as a pdf and uploaded.<\/em><\/li>\n<li><em><strong>AGAIN THIS YEAR! Coggins papers for each horse must be scanned and uploaded to 4-H Connect during the entry process. It is best if the dates of the test are good THROUGH the dates of the Texas 4-H Horse Show. **Please note that your entry will not be considered complete until we have a current Coggins Test on file.
