Scan Forms for Outcome Programs – Tests of Knowledge

Use these forms to create your own test of knowledge using true-false or multiple choice. Simply enter the question text and responses for each question.

Please note that these forms only include test of knowledge questions – nothing else. However, you have some options on the number of questions to include on the test (see details below).


STEP-BY-STEP INSTRUCTIONS
:

  1. Develop your test of knowledge in Word – questions and multiple choice items (if applicable) – before using the scan form.
  2. If you develop questions for just one side of the form, use the toggle on the 2nd page (this will blank out unused response categories but leave the survey id and four cornerstones).
  3. Print two copies. Be sure to print all pages of the forms – even blank page(s) – so that the four cornerstones appear on every page. 
  4. In the “Office Use Only” box, fill in the pre-bubble on one form; fill-in the post-bubble on the other form.
  5. Make copies of both the pre and post tests (copy two-page forms on single sheet).
  6. Send a key with the correct answers with your completed forms and cover sheet.


TRUE-FALSE

 

With these forms you can create a test of knowledge using true-false questions on a single sheet of paper. Depending on the form, the number of questions can vary from a minimum of 10 to a maximum of 20.

 

  • For 10 questions – Use this form (1 page, front side only). Fill-in questions for questions 1 – 10. Print two copies. Mark pre or post in the office use only.
  • For 20 questions – Use this form (1 page, front and back sides). Fill-in questions for all 20 questions. Print two copies. Mark pre or post in the office use only.
  • For 11 – 19 questions – Use this form (1 page, front and back sides). Fill-in questions and possible responses. Print two copies; then use a sheet a paper to cover unused questions on the back side while making copies. CAUTION: Do not cover any of the four cornerstones or form id (they must intact to remain scannable). Print two copies. Mark pre or post in the office use only.


MULTIPLE CHOICE

With these forms you can create a test of knowledge using multiple choice questions. Depending on the form, the number of questions can vary from a minimum of 10 to a maximum of 20. All questions provide for 4 possible responses.

  • For 10 questions – Use this form (1 page, front and back sides). Fill-in questions and possible response for questions 1 – 10. Print two copies. Mark pre or post in the office use only.
  • For 12 questions – Use this form (1 page, front and back sides on each page). Fill-in questions and possible response for all 20 questions. Print two copies. Mark pre or post in the office use only.
  • For 20 questions – Use this form (2 pages, front and back sides on each page). Fill-in questions and possible response for all 20 questions. Print two copies. Mark pre or post in the office use only.
  • For 13 – 19 questions – Use this form (2 pages, front and back sides). Fill-in questions and possible responses. Print two copies; then use a sheet a paper to cover unused questions on the page 2 while making copies. CAUTION: Do not cover any of the four cornerstones or form id (they must intact to remain scannable). Mark pre or post in the office use only.
