Important Notes: All forms on this page meet the state requirement for reporting customer satisfaction. Formerly these were legal size and were called “customer satisfaction extended forms.” They have been reduced to letter size and renamed to “customer satisfaction plus client change forms.”

These forms have the state-required questions for customer satisfaction plus one or more sections for client change.

Change in understanding is typically measured at the conclusion of a one-time program but can be used with a program series as well:

All legal-size customer satisfaction forms have been discontinued and replaced with a letter-size version above. Organizational Development will continue to accept legal size forms that have already been used to collect to data but please use the new, letter size forms for new program events.
