You will need to activate the Gravity Form Widget
Choose Plugins,
Find Gravity Forms, Check the box, Scroll down to bottom of page and
Click on arrow in the Bulk Actions, choose Activate, Apply
The new Plugin will show up on the left menu
Under Forms, Choose New Form
Place cursor on Untitled Form
Click on Edit
Type Title in Tile Box (Untitled Form)
In the Description box you can change it to say what you want.
Click on the top Confirmation tab
Here you can change the text in the box again to say what you want. People filling out the form will get this message when they hit the submit button
Click arrow on the Standard Fields

Click on Single Line Text
Choose Edit, Type in Field Label
Choose required field or NOT
To change width size of line (to allow for more text)
Click the Advanced tab
Click Arrow in Field Size and choose Small, Medium or Large

Click in box next to Hide Country Field
Click in box next to State/Province/Region Field
Click in box next to Hide Address Line 2 Field

This allows them to type in their website address
No Editing required unless it’s a REQUIRED filed
Click on Edit to make field Required or NOT
Email
This allows them to type in their Email Address
No Editing required unless it’s a REQUIRED filed

Choose file upload, Edit
Give it a Field Label name
In the Allow file extension you can add jpg, gif, pdf
NOTE: these are the only files allowed in wordpress
Click on Edit to make field Required or NOT
