FY2023 District 4 Regional Blanket Travel Authorization

Out of District Travel Authorization – AG-802 To be completed for any extension related travel that takes you out of District 4 or District 5.  Authorization form must be submitted to the district office for Hurley’s approval.

Out of State/Country Travel Authorization – To be completed for any extension related travel that takes you out of Texas or the United States.  Authorization form must be submitted to the District Office for Hurley’s approval 30 days prior to the first day of departure, and then on to Dr. Jeff Ripley for his approval.
